Job summary
On Lok Inc. in San Jose, CA seeks a licensed Physical Therapist to provide and supervise therapy for participants in the Day Health Center and at home. You will evaluate, plan and administer PT treatment under supervision, and mentor staff including rehab aides.
The position requires CA PT licensure, a degree in Physical Therapy, and 1–2 years of experience with elderly or frail populations. You will join an interdisciplinary team dedicated to quality person-centered care.
Qualifications
- Graduate of an approved university course in Physical Therapy.
- Licensed by the California Physical Therapy Board.
- Minimum one to two years working with elderly or frail populations.
- Able to work in a multidisciplinary team and be flexible.
- Computer literate.
Responsibilities
- Evaluates participants and develops a measurable PT plan.
- Completes initial and re-evaluation reports and tracks progress.
- Supervises PT assistant and rehab aide; provides training as needed.
- Participates in interdisciplinary team planning and problem solving.
- Other related duties as assigned.
