Fox Rehabilitation is seeking licensed Physical Therapists to provide one-on-one geriatric home care in Nashua, NH, delivering evaluations, individualized plans, and evidence-based therapy in private residences and senior living communities.

Clinicians may incorporate telehealth and virtual care for greater flexibility and continuity of care, building strong patient relationships with autonomy and meaningful outcomes.

Formación

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilidades

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and quality of life.
• Document in the EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
