FOX Rehabilitation, a clinician-operated practice delivering Geriatric House Calls™, seeks licensed Physical Therapists to provide one-on-one care to older adults in Nashua, NH. Therapists manage their own schedule, treating patients in private residences or senior living communities while incorporating telehealth when appropriate.

Responsibilities include evaluations, treatment planning, patient education, and EMR documentation.

Competenze

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Mansioni

• Provide one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and safety.
• Document effectively within the EMR in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote aging in place and safety.
• Provide telehealth / virtual care when appropriate to extend access.
