Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one care in Nashua, NH, either in private homes or senior living communities. You will craft personalized treatment plans to enhance mobility and independence, with flexibility to incorporate telehealth and virtual visits when suitable.

Qualifications

• Licensed Physical Therapist in NH.
• DPT or equivalent degree from an accredited program.
• Proficient with EMR systems and documentation.

Responsibilities

• Provide one-on-one PT to older adults in homes or senior living communities.
• Improve mobility, independence, and quality of life.
• Document sessions accurately in the EMR per regulatory standards.
• Collaborate with interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Incorporate telehealth when appropriate to extend access.
