FOX Rehabilitation in Elizabeth, NJ is seeking a licensed Physical Therapist to deliver one-on-one care in patients’ homes or senior living communities. You’ll evaluate patients, create individualized treatment plans, and provide evidence-based therapy to improve mobility and independence.

The position offers autonomy with scheduling, potential telehealth integration, and a clinician-driven culture focused on meaningful outcomes for older adults in the community.

Formación

• Licensed PT in the state of practice or eligible to obtain license.
• Graduated from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilidades

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence, and function.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients and caregivers to promote safety and aging in place.
