Job summary

FOX Rehabilitation is hiring a Physical Therapist to deliver in-home and senior living care in Nashua, NH. You will assess, plan and execute individualized therapy programs for older adults, with emphasis on mobility, independence and quality of life.

The role offers autonomy, flexible scheduling and the option to incorporate telehealth as part of patient care. A licensed NH PT with EMR proficiency is required.

Qualifications

• Licensed Physical Therapist with valid license in NH or eligibility to obtain it.
• Experience in geriatric population and home-based care is preferred.
• Proficient with electronic medical records (EMR) and telehealth tools.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Deliver therapy sessions to improve mobility, independence and functional outcomes.
• Document treatment plans and progress in the EMR per regulatory standards.
