Job summary

FOX Rehabilitation is seeking licensed Physical Therapists in Nashua, NH to deliver one-on-one in-home or Senior Living care under our Geriatric House Calls model. You’ll assess, treat, and monitor older adults in private residences or communities, building strong patient relationships while maintaining autonomy over your schedule.

This role emphasizes evidenced-based care, collaboration with PT/OT/SLP teams, and occasional telehealth to extend access.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, etc.).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document effectively within FOX's EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
