Job summary

Legacy Healthcare seeks a dedicated Physical Therapist (PT) to join its PACE program in the Louisville Metro area, serving older adults in both home and center environments.

You will evaluate mobility, develop individualized care plans, and provide therapeutic interventions to enhance independence while reducing fall risk. Collaboration with RNs, PCPs, OTs, and social workers is essential.

Qualifications

• Licensed Physical Therapist (PT) in Indiana or eligible for licensure.
• Minimum 1 year of geriatrics/home health/SNF/outpatient/PACE experience preferred.
• Excellent interpersonal and communication skills in an interdisciplinary model.
• Proficient in EMR systems and documentation best practices.
• Valid driver’s license and reliable transportation for home visits.

Responsibilities

• Evaluate and assess participants’ physical function, mobility, gait, balance, and rehab needs in home and center settings.
• Develop, implement, and modify individualized PT care plans with the interdisciplinary team.
• Provide therapeutic interventions to improve or maintain mobility and reduce fall risk.
• Instruct participants, caregivers, and families in exercises, mobility techniques, and assistive devices.
• Document assessments, treatments, and progress per PACE standards and regulatory guidelines.
• Coordinate with RNs, PCPs, OTs, social workers, and other team members in care planning meetings.
