AltaMed Health Services Corp. in the United States is seeking a Physical Therapist–PACE to provide high‑quality services that restore function, mobility, relieve pain, and promote overall health. You will evaluate mobility dysfunction and develop individualized care plans aligned with patient goals within the Rehab team.

The role supports training, data collection, CQI initiatives, and inter‑disciplinary collaboration, with remote or in‑person duties as needed to support AltaMed PACE centers.

Qualifications

• Licensed Physical Therapist with state licensure (CA preferred).
• Current BLS certification required.
• Bilingual abilities in English and additional languages preferred.

Responsibilities

• Evaluate mobility dysfunction and design individualized care plans.
• Deliver PT services remotely or in‑person for AltaMed PACE centers.
• Assist with onboarding, training, and competency re‑training as directed by Rehab Manager.
• Support CQI initiatives, data collection, and report preparation.
• Collaborate with interdisciplinary teams to optimize participant health outcomes.
