Job summary

AltaMed Health Services Corp. seeks a Staff Physician to deliver comprehensive primary care within the Program of All-Inclusive Care for the Elderly (PACE).

You will work with the Interdisciplinary Team to provide patient-centered, regulatory-compliant care across the PACE center, participants' homes, virtual visits, and SNFs. You will manage a defined panel, promoting wellness and coordinating care with specialists and IDT members.

Qualifications

• Board-certified and eligible in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics.
• Active unrestricted CA license to practice medicine.
• DEA license is required.
• BLS certification (or higher) is required.
• EMR experience preferred.

Responsibilities

• Deliver comprehensive primary care within the PACE program.
• Collaborate with the Interdisciplinary Team (IDT) to ensure high-quality, regulatory-compliant care.
• Manage a defined panel of participants across the PACE center, homes, and SNFs.
• Coordinate care with specialists and IDT members to promote wellness.
