Job summary

Therapy Management Corporation (TMC) is seeking a licensed Physical Therapist to serve adults and geriatrics, delivering patient-centered care to improve independence, mobility, safety and quality of life. The role emphasizes clinical autonomy within a collaborative clinical environment.

You’ll evaluate and treat patients, develop individualized plans, and coordinate with interdisciplinary teams while maintaining compassionate care and a commitment to outcomes in a supportive setting.

Qualifications

• Licensed PT or license eligible in the practicing state.
• Graduate of an accredited PT program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance, and functional independence.
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals.
• Deliver hands-on therapy interventions to support recovery, increased endurance, and enhanced quality of life in older adults.
• Help patients restore functional mobility and safely perform daily activities with greater confidence and independence.
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals.
