Qualifications

• Licensed Physical Therapist (PT) or license eligible in the state of practice.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long‑term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy to improve mobility and independence.
• Develop and implement patient-centered treatment plans with long-term goals.
• Deliver hands-on therapy to support recovery and enhance quality of life in older adults.
• Collaborate with interdisciplinary teams, caregivers, and nursing staff to drive outcomes.
• Track progress, document accurately, and adapt plans as needed.
• Maintain continuity of care and ensure safety in daily activities.
