Job summary

Therapy Management Corporation (TMC) is seeking a licensed Physical Therapist to empower older adults in adult/geriatric rehabilitation. You will evaluate patients, design personalized treatment plans, and deliver hands-on therapy to improve mobility and independence in a collaborative clinical environment.

Qualifications

• Licensed PT or eligible for state licensure.
• Graduated from an accredited Physical Therapist program.
• Interest or experience in skilled nursing, long-term care or geriatric rehab.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy to improve mobility and independence.
• Develop and implement patient-centered treatment plans focused on long-term goals.
• Deliver hands-on interventions to support recovery and endurance in older adults.
• Help patients regain functional mobility for daily activities with confidence.
• Collaborate with interdisciplinary teams and caregivers to ensure continuity of care.
• Track progress and maintain accurate clinical documentation and adapt plans as needed.
