Therapy Management Corporation (TMC) is seeking a Physical Therapist to empower adult and geriatric patients through compassionate, patient-centered care. You will work in a collaborative clinical environment to improve independence, mobility, safety and overall quality of life, while maintaining clinical autonomy and delivering high-quality rehabilitation services.

Qualifications

• Licensed PT in the state of practice (or eligible).
• Experience or interest in geriatric rehabilitation and skilled nursing settings.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility and functional independence.
• Develop and implement patient-centered treatment plans with long-term goals.
• Deliver hands-on therapy interventions to support recovery and quality of life in older adults.
• Help patients restore functional mobility and daily activities with confidence and safety.
• Collaborate with interdisciplinary teams to drive meaningful outcomes and continuity of care.
• Track progress, maintain accurate documentation, and adapt plans as needed.
