Qualifications

• Licensed PT in the state of Minnesota or eligible for licensure
• Graduate of an accredited Physical Therapist Program
• Interest or experience in geriatric rehabilitation
• Passion for patient-centered care and improving quality of life for older adults

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance, and functional independence
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals
• Deliver hands-on therapy interventions to support recovery and enhanced quality of life in older adults
• Help patients restore functional mobility and safely perform daily activities
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans as needed
