Job summary

Telos Health Systems, Inc. seeks a Psychotherapist in Fulton, KY to provide mental health evaluations and psychotherapy in SNF/AL settings. The role offers flexibility, clinical autonomy, and strong administrative support.

Licensure in LCSW/LPCC/LMFT or as a Psychologist (PsyD/PhD) is required, with full-time equivalence possible and a sign-on bonus up to $7,500. Hybrid in-state work model supports clinician needs.

Qualifications

• Active Kentucky license as one of the listed credentials.
• Master’s degree in a relevant mental health field for LCSW/LPCC/LMFT; Doctoral for Psychologist.
• Ability to work independently and with a clinical team.
• Strong written and verbal communication and clinical documentation skills.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with a multidisciplinary team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff and other healthcare professionals.
• Adhere to federal, state, and organizational standards and policies.
