Curana Health is seeking a seasoned Psychiatric Nurse Practitioner to deliver high-quality mental health services across SNF, LTC, AL, and IL communities. You will join a mission-driven team focused on compassionate geriatric behavioral health and continuity of care across post-acute settings.

The role emphasizes collaborative care, autonomous practice with strong clinical support, and opportunities to impact seniors’ well-being through data-driven, patient-centered approaches.

Qualificações

• Graduation from an accredited Nurse Practitioner program and ANCC PMHNP certification.
• Active DEA license and BLS/ACLS certifications.
• Current NP license in the state of practice.

Responsabilidades

• Psychiatric care and rounding in SNF, LTC, AL, and IL settings.
• Collaborative care coordination with attending physicians, consulting specialists, referral sources, and facility staff.
• Assess and manage residents' mental health needs, perform physical exams, and gather complete medical histories.
• Order and interpret lab and diagnostic tests, develop effective treatment plans for mental and physical health.
• Prescribe and administer medications and treatments; provide injections as necessary.
• Educate patients and families on medications, care plans, and preventive mental health practices.
• Work with physicians and facility teams to address acute psychiatric and medical needs promptly.
