Job summary

Curana Health is seeking an experienced Psychiatric Nurse Practitioner to join our growing team and deliver high-quality mental health services across SNF, LTC, AL, and IL communities.

You will work with a multidisciplinary team to develop personalized treatment plans, perform comprehensive evaluations, and ensure continuity of care across post-acute and long-term care settings, with a focus on geriatric psychiatry and compassionate care.

Qualifications

• Graduation from an accredited Nurse Practitioner program and ANCC certification as a PMHNP.
• Active DEA license.
• BLS and ACLS certification.
• Current Nurse Practitioner license in the state of practice.

Responsibilities

• Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
• Collaborative Care Coordination with attending physicians, consulting specialists, referral sources, and facility staff.
• Assess and manage the mental health needs of residents, perform physical exams, and gather medical histories.
• Order and interpret lab and diagnostic tests, develop effective therapeutic care plans.
• Prescribe and administer medications and treatments; provide injections as necessary.
• Educate patients and families on medications, care plans, and preventive mental health practices.
• Work alongside physicians and facility teams to address acute psychiatric and medical needs promptly.
