CareSource in Wisconsin seeks a physician to deliver primary medical care to Community Care Program members, under supervision of the Medical Director. You will assess, plan, and provide direct care while collaborating with an interdisciplinary team to ensure effective outcomes.

The role emphasizes geriatrics for at-risk populations, teamwork with clinicians, and ongoing quality improvement and member education within a patient-centered culture.

Competenze

• Minimum 1 year recent primary care experience with an at risk or elderly population.
• Board certification in Internal Medicine or Family Practice required; geriatrics certification preferred.
• Strong communication and teamwork with interdisciplinary care teams.

Mansioni

• Obtains a complete health history and records findings, and maintains documentation of health care services.
• Performs physical examinations, functional status evaluation and/or mental status evaluations as indicated. Treats acute and chronic conditions based on member history, clinical assessment, laboratory and diagnostic testing, and member preferences and goals of care.
• Provides preventive health care, episodic care and health promotion for members, and arranges referrals as indicated.
• Regulates and adjusts medications as needed.
• Educates members and families regarding health maintenance and acute and chronic illnesses.
• Functions as a member of the interdisciplinary team and participates in the development of the comprehensive plan of care and ongoing monitoring of members health status, providing geriatric and medical knowledge when needed.
