Job summary

Center for Elders Independence is a California-based PACE organization providing comprehensive, integrated care for elderly participants. The Primary Care Physician will lead medical care, guide the interdisciplinary team, and ensure care plans align with participant goals within a collaborative, community-focused setting.

The role emphasizes coordination with family caregivers, after-hours on-call support, and close oversight of care plans, tests, and referrals to optimize outcomes for elderly

Qualifications

• MD with California medical license; CME requirements to renew license.
• Minimum 1 year experience working with frail/elderly population.
• Geriatrics preferred; interest in PACE interdisciplinary team.
• Board certification in Internal Medicine or Family Practice.
• DEA certificate and BLS certification required.
• Strong communication, leadership and team collaboration in a diverse setting.

Responsibilities

• Provide comprehensive medical care guided by the PACE benefit.
• Conduct initial, semi-annual, annual and periodic assessments.
• Develop and implement participants' care plans with the IDT.
• Review and sign care plans every 6 months.
• Supervise CEI nurse practitioners and physician assistants as appropriate.
• Coordinate diagnostics, follow-ups and referrals with families.
• Provide after-hours on-call telephonic medical services.
• Document in EMR and participate in provider/clinic meetings.
