Oak Street Health is hiring a Primary Care Physician to deliver equity-focused, value-based care to local Medicare populations across our neighborhood centers. You will work with large care teams to improve outcomes and patient experience, prioritizing quality over volume in a supportive, mission-driven environment.

Qualifications

• M.D. or D.O. degree and board certification (or board eligible) in Internal Medicine or Family Medicine.
• Active, unrestricted State Medical License in the US.
• Active DEA license and US work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee and administer primary care within a value-based care model.
• Prescribe medications and manage treatment plans.
• Maintain and update Patient Electronic Medical Records.
• Collaborate with leadership to meet quality and outcome goals.
• Work with center care teams to direct operations and improve patient experience.
