Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to Medicare populations at our Gary, Indiana center. This full-time role combines clinical leadership with a collaborative care model supported by a large team, focusing on quality outcomes over volume.

The position offers a generous salary range of $240,000 to $270,000 plus incentives, with comprehensive benefits. New graduates are welcome; experienced candidates with geriatrics or value-based care

Qualifications

  • Must be MD or DO with board certification in Internal or Family Medicine (or board-eligible).
  • Active, unrestricted State Medical License and DEA license.
  • US work authorization required.

Responsibilities

  • Assess and diagnose patients at our local clinics.
  • Oversee, direct, and administer primary care.
  • Prescribe and administer pharmaceutical treatments and medication.
  • Maintain Patient Electronic Medical Record data via canopy and greenway.
  • Collaborate with regional leadership to meet health quality goals.
  • Direct and manage the center care team with Practice Managers.
  • Perform other duties as assigned.
