Job summary

Oak Street Health is seeking a Primary Care Physician who will deliver value-based healthcare to local Medicare patients. Candidates should have a Medical Doctor or Doctor of Osteopathy degree and preferably Board Certification in Internal Medicine or Family Medicine.

This role involves overseeing patient assessments, collaborating with healthcare teams, and managing care processes at innovative care centers located in Connecticut. Strong preferance is given to candidates experienced in geriatrics.

Qualifications

  • Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate is required.
  • Board Certification in Internal or Family Medicine is preferred.
  • Active, non-probationary, unrestricted State License is mandatory.

Responsibilities
