Job summary

Oak Street Health is seeking a Primary Care Physician to provide equitable, value-based care at our network of neighborhood centers in Colorado. This part-time role focuses on delivering quality care to Medicare populations with a dedicated care team.

Responsibilities include assessing patients, guiding treatments, maintaining EMR data, collaborating with leadership, and supporting center operations. A MD/DO with board eligibility and active licensure is required; bilingual skills are a plus.

Qualifications

• MD or DO required; board certification in Internal/Family Medicine or eligibility.
• Active, unrestricted State Medical License and DEA license required.
• US work authorization is required to apply.

Responsibilities

• Assess and diagnose patients at our local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient EMR data via canopy and Greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
