Job summary

Oak Street Health is seeking a Primary Care Physician to provide equitable, value-based care to local Medicare populations across our network of neighborhood clinics. Our physicians work within supportive care teams to deliver high-quality outcomes with a focus on prevention and patient experience.

Ideal candidates are MD or DO, board certified or board eligible, with an active license and DEA, and a commitment to collaborative, patient-centered care. New graduates welcome.

Qualifications

• MD or DO degree from an accredited medical school.
• Board Certification in Internal Medicine or Family Medicine or board eligible.
• Active, unrestricted State Medical License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient Electronic Medical Record data.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
