Job summary

Oak Street Health in Commerce City, CO is seeking a Part-time Primary Care Physician to deliver value-based care to local Medicare populations. Work as part of a neighborhood center team dedicated to high-quality, patient-centered outcomes.

Responsibilities include assessing and diagnosing patients, managing primary care services, maintaining EMR data, collaborating with leadership, and guiding the care team.

Qualifications

• MD or DO degree required.
• Board certification in Internal Medicine or Family Medicine (or board eligible).
• Active, unrestricted State License, and US work authorization.

Responsibilities

• Assess and diagnose patients at our local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain Patient Electronic Medical Record data via canopy and greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned
