Job summary

Oak Street Health is seeking a Part-time Primary Care Physician in Colorado to deliver equitable value-based care to local Medicare populations within our neighborhood clinics. You will join a team-focused environment designed to reduce hospital admissions and emphasize preventive medicine, with a compensation model that rewards outcomes.

New graduates encouraged to apply; Center Medical Director positions may be considered for highly qualified candidates.

Qualifications

• MD or DO graduate required.
• Board Certification in Internal Medicine or Family Medicine (or board eligible).
• Active, non-probationary, unrestricted State License required.
• Active DEA license and US work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain Patient EMR data via Canopy and Greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
