CenterWell / Conviva Primary Care, part of Humana Inc., is seeking a Primary Care Physician for a brand-new clinic in Port Salerno/Delray Beach, FL. The role emphasizes hands-on patient care and understanding social determinants of health to improve outcomes.

Onsite and in-person at Conviva Delray Beach Center, with a 40-hour workweek and a focus on direct patient care, including extended appointment times for seniors.

Qualifications

• Doctor of Medicine or Osteopathic Medicine.
• Board certified or board eligible in Geriatric, Family, or Internal Medicine.
• Valid, unrestricted license in the state of employment (or willing to obtain).
• Current, unrestricted DEA license.
• TB Screening required if selected.

Responsibilities

• See patients on a daily basis with 40-hour work week.
• Spend time in direct patient care in clinic with longer appointment times.
• Report to Associate Medical Director and work onsite at Conviva Delray Beach Center.
