Job summary

WelbeHealth is seeking a Primary Care Provider to deliver continuous medical coverage for participants in the PACE program. You will perform in-person assessments and collaborate with WelbeHealth physicians and the IDT to plan primary care and educate participants and families.

This role features a smaller patient caseload to foster long-term relationships, with opportunities to practice value-based care. Benefits include base pay plus bonus, 401(k) match, CME reimbursement, and comprehensive

Qualifications

• Graduation from an accredited school of nursing.
• Certificate as a Nurse Practitioner or Physician Assistant with prescriptive authority.
• RN state license.
• Minimum of two years of experience with frail or elderly populations.
• Knowledge of chronic care and geriatric best practices.

Responsibilities

• Provide continuous medical coverage and comprehensive in-person assessments.
• Collaborate with physicians and IDT to develop care plans and coordinate care.
• Visit participants in skilled nursing facilities and residences as needed.
• Review outcomes and utilization data to improve practices and policy.
