Job summary

BoldAge PACE in North Charleston, SC is seeking a PCP Nurse Practitioner to deliver primary care and continuous healthcare services to PACE participants, collaborating with the clinical care team to assess, plan, and provide care in alignment with PACE Program standards.

You will conduct assessments, reassessments, develop care plans, manage medications, and ensure safe transitions across care settings while actively participating in daily IDT meetings and on‑call duties.

Qualifications

• MD/DO/NP/APRN/PA with active licensure.
• State CDS Certification and DEA Registration.
• BLS card within 90 days of employment.
• 2 years' direct patient care experience preferred.
• 1 year experience with frail or elderly population preferred; training provided if needed.

Responsibilities

• Provide high-quality primary care with a strong focus on individualized evaluation, starting with a comprehensive history and physical exam, rather than broad diagnostic testing panels.
• Collaborate closely with the IDT before referring participants to specialists, ensuring referrals are necessary and continuity of care is maintained.
• Collaborate with the Medical Director to manage specialist referrals, initiate treatment prior to referral when indicated, review notes promptly, and develop care plans with clinical judgment.
• Manage and monitor participants' medications, aiming for quality reconciliation and safe interactions with support from the Medical Director and pharmacy vendor.
• Stay informed of participant status during hospitalizations and facility stays to ensure safe transitions and continuity of care.
• Assume shared responsibility for all participants, supporting the IDT model of team-based care.
• Participate in daily IDT meetings, contributing clinical insights and participant-centered care plans.
• Ensure timely completion of assessments and documentation in line with policy, regulatory, and quality standards.
• Review labs, imaging, and consults and communicate changes to the IDT.
• Educate participants and caregivers on self-care, medication use, and wellness practices.
• Use the ED judiciously, prioritizing PACE resources unless life-threatening.
