Job summary

BoldAge PACE is seeking a clinician to deliver primary care and continuous health services to our participants, coordinating with the clinical care team to assess needs and develop care plans in alignment with PACE standards.

You will perform comprehensive history and physicals, monitor medications, work with the IDT to coordinate referrals, and participate in 24/7 on-call coverage while supporting safe transitions and quality outcomes for frail older adults served at home and in our centers.

Qualifications

• MD/DO/NP/APRN/PA required.
• NJ NP certification per NJ State Board of Nursing.
• Physician licensed in NJ or authorized by NJ Medical Examiners.
• State CDS Certification and DEA Registration.
• Board Certified in Geriatrics preferred.
• BLS required within 90 days of employment.
• 2 years direct patient care experience preferred.
• 1 year experience working with a frail or elderly population preferred; training provided if applicable.

Responsibilities

• Provide high-quality primary care with a focus on individualized evaluation and comprehensive assessment.
• Collaborate with the IDT to coordinate referrals and develop participant care plans.
• Manage and monitor medications with input from Medical Director and Pharmacist vendor.
• Ensure safe transitions and care continuity during hospitalizations or facility stays.
• Participate in daily IDT meetings and contribute to participant-centered care plans.
• Complete participant assessments and documentation compliant with policy and quality standards.
• Educate participants and caregivers on self-care strategies and wellness practices.
• Provide 24/7 on-call coverage and effective hand-offs.
