Job summary

WelbeHealth PACE is seeking a Primary Care Provider (NP/PA) in San Jose, CA to deliver comprehensive in-person assessments and care planning for elderly participants within a value-based care model.

You will collaborate with WelbeHealth physicians and the IDT to develop and coordinate participant care, discharge planning, and ongoing disease management, while maintaining a smaller caseload for deeper patient relationships.

Qualifications

• Graduation from an accredited school of nursing and advanced practice licensure.
• Certification as a Nurse Practitioner or Physician Assistant with prescriptive authority.
• Minimum of two years’ experience with frail or elderly populations.
• Knowledge of chronic care and geriatric issues.
• Reliable transportation and willingness to rotate night and weekend call.

Responsibilities

• Provide ongoing medical coverage and comprehensive assessments (initial, semi-annual, unscheduled).
• Develop participant care plans with the IDT and ensure care coordination.
• Support in skilled nursing facilities and residences with regular visits as needed.
• Review outcomes and utilization data to drive best practices and policy improvements.
