Job summary

UPMC Central PA is seeking Per Diem adult and senior care physicians to join our growing post-acute care team in the Lancaster/Lititz region. This role involves care across skilled nursing facilities and clinics, emphasizing geriatric care and team collaboration.

Clinicians will interact with leadership and care teams, work with Advanced Practice Providers, and participate in M-F rounds with flexible scheduling. Overnight and weekend call is shared among partners with favorable ratios.

Qualifications

• Board certified/board eligible in Internal, Family or Geriatric Medicine.
• Team oriented with the ability to provide personalized patient care and demonstrate clinical outcomes.
• Commitment to providing high-quality and compassionate care to patients.
• Eligible for licensure in the state of Pennsylvania.

Responsibilities

• Provide direct patient care across skilled nursing, rehab, personal care/assisted living, independent living and clinic settings across multiple facilities.
• Collaborate with facility leadership, hospital discharge team, care management and other specialties to drive quality care.
• Supervise and collaborate with Advanced Practice Providers.
• Clinical rounding M-F with flexible schedules; overnight and weekend call shared among partners and APPs.
