Job summary

UPMC Central PA is seeking full-time Adult and Senior Care Physicians to join our Post-Acute Care team across multiple skilled nursing facilities in the Hanover region of South Central Pennsylvania.

Provide direct patient care across aging continuum, collaborate with leadership and discharge teams, and utilize EPIC EMR. Flexible schedules with call coverage shared among partners and APPs, supporting work-life balance.

Qualifications

• Board certification or eligibility in Primary Care, Internal Medicine, Family Medicine or Geriatrics.
• Team oriented with the ability to deliver personalized patient care and demonstrate outcomes.
• Commitment to high-quality, compassionate care and licensure in PA.

Responsibilities

• Provide direct patient care in multiple facilities across aging continuum (skilled nursing, rehab, assisted living, clinic).
• Collaborate with facility leadership, discharge teams, care management and other specialties to drive quality care.
• Supervise Advanced Practice Providers and use EPIC EMR for orders, diagnoses, and tests.
• Clinical rounding on weekdays with favorable on-call coverage shared with partners and APPs.
