Job summary
UPMC Central PA is seeking full-time Adult and Senior Care Physicians to join our Post-Acute Care team in the Hanover region. You will provide direct patient care across skilled nursing facilities and clinics, focusing on geriatric medicine.

Qualifications
- Board-certified/board-eligible in Primary Care, Internal Medicine, Family Medicine, or Geriatric Medicine.
- Experience collaborating with facility leadership and care teams is valued.
- Experience with EPIC EMR and interfacility coordination is preferred.
Responsibilities
- Provide direct patient care across multiple facilities including skilled nursing, rehab, and clinics.
- Collaborate with facility leadership, discharge teams and care management to ensure high-quality care.
- Supervise Advanced Practice Providers and coordinate with other specialties.
- Utilize EPIC EMR and PACS for ordering and diagnostics.
- Support flexible scheduling with balanced call coverage.
