Job summary

UPMC Central PA is seeking full-time Adult and Senior Care Physicians for a growing Post-Acute Care team in the Hanover region of South Central Pennsylvania. You’ll provide direct patient care across multiple facilities, collaborate with leadership and care teams, and supervise APPs in a supportive health system.

Qualifications

• Board certified/Board eligible in Primary Care or Internal, Family or Geriatric Medicine.

Responsibilities

• Provides direct patient care across aging continuum (skilled nursing, rehab, personal care, independent living, clinic).
• Collaborates with facility leadership, discharge teams and care management to drive quality.
• Supervises Advanced Practice Providers and coordinates with multiple facilities.
• Uses EPIC EMR and online diagnostics to support clinical decisions.
