Job summary

Access Healthcare Associates is seeking a compassionate Mobile Primary Care Nurse Practitioner or Physician Assistant to provide in-home and senior living care. This role supports aging adults with comprehensive assessments, care planning, and chronic-condition management, all delivered with a patient-centered approach.

You will work within Brigade Health and its Affiliates to extend medical expertise directly to homes and communities, with opportunities for growth and professional advancement

Qualifications

• Active, unencumbered NP or PA license in the state you will be practicing.
• Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
• Compassionate, patient-centered approach and strong clinical judgment.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver personalized, high-quality care to older adults in homes and senior living settings.
• Conduct comprehensive assessments and develop individualized care plans.
• Manage chronic conditions with skill and empathy.
• Collaborate across clinical disciplines (primary, psych, palliative, neuro) for whole-person care.
• Build trusted relationships with residents, caregivers, and community partners.
