Curana Health is seeking an experienced Psychiatric Nurse Practitioner (PMHNP) to join our team and provide high-quality mental health services across SNF, LTC, AL, and IL communities. You will work with a multidisciplinary team to ensure continuity of care in post-acute and long-term settings.

This full-time role requires travel between facilities. We offer a collaborative environment, clinical and administrative support, and opportunities for professional growth.

Qualifications

• Graduation from an accredited Nurse Practitioner program and ANCC certification as a PMHNP.
• Active DEA license.
• BLS and ACLS certification.
• Current Nurse Practitioner license in the state of practice.

Responsabilités

• Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
• Collaborate with physicians, specialists, and facility staff for coordinated care.
• Assess and manage mental health needs, perform exams and histories.
• Order and interpret labs, develop treatment plans for mental and physical health.
• Prescribe medications and provide patient and family education.
