Qualifications

• PMHNP with ANCC certification and active license.
• DEA license active.
• BLS and ACLS certification.
• Current NP license in the state of practice.

Responsibilities

• Conduct patient rounds and manage psychiatric care across SNF, LTC, AL, and IL settings.
• Partner with attending physicians, consulting specialists, referral sources, and facility staff for coordinated care.
• Assess and manage residents' mental health needs; perform physical exams and history gathering.
• Order and interpret labs/tests; develop therapeutic care plans for mental and physical health.
• Prescribe and administer medications and treatments; provide injections as needed.
• Educate patients and families on medications, care plans, and preventive mental health practices.
• Collaborate with care teams to promptly address acute psychiatric and medical needs.
