Job summary

The Center for Elders’ Independence in Oakland, CA seeks a Physical Therapist to provide restorative and rehabilitative services within the interdisciplinary care team. The role supports the total plan of care for participants in PACE settings.

Under supervision, you will evaluate, treat, and educate participants and families, document outcomes, and coordinate with clinicians and vendors to ensure appropriate equipment and services are delivered.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current license as a Physical Therapist in California.
• CPR and First Aid certification on file.
• Valid California driver’s license and reliable vehicle.
• Ability to travel to multiple work sites and participant residences.

Responsibilities

• Collaborates with care teams to evaluate participants and develop care plans.
• Instructs participants and caregivers on mobility devices usage.
• Works with vendors to obtain rehabilitative equipment and supplies.
• Educates team and family on the participant’s PT program.
• Treats participants to relieve pain and restore function per plan of care.
• Performs initial assessments and periodic reassessments.
• Documents progress and communicates changes to the team.
• Supervises therapy aides and provides in‑service training.
• Participates in team conferences and plan development.
• Assesses home environments and makes safety recommendations.
• Maintains certifications and participates in required trainings.
• Supports 24‑hour care coordination and delegates to PTAs as appropriate.
• Maintains a clean, organized department and safe equipment.
• Keeps patient information confidential and follows safety procedures.

Education

Bachelor’s degree in Physical Therapy

Licensed Physical Therapist (CA)

CPR and First Aid certification

Driver’s license

Ability to travel to multiple sites
