Job summary

BoldAge PACE seeks a dedicated Physical Therapist to plan, organize, and deliver PT services for our participants. You will assess participants, plan treatments, and educate families to support home-based, community-focused care.

Responsibilities include in-person assessments at enrollment, collaboration with the IDT, and documentation of care plans. OT/PT staff training and safety guidance are also part of the role in a center-based and home-visit environment.

Qualifications

• Advanced degree in Physical Therapy from an accredited school.
• Current license to practice Physical Therapy in the state.
• Two years of experience on an interdisciplinary team in a hospital, nursing home or community-based setting preferred.
• 1 year of experience with a frail or elderly population preferred; training provided if not present.
• Valid driver's license and proof of auto insurance.

Responsibilities

• Plans, organizes, and implements Physical Therapy services for PACE participants.
• Performs in-person assessments for participants at initial enrollment and ongoing as determined by the IDT.
• Assesses need for adaptive equipment, specialized shoes and prosthesis; evaluate home safety and mobility; recommend home modifications and equipment.
• Develops and implements physical therapy treatment programs; educates participants, caregivers, and families.
• Trains PACE staff in body mechanics, equipment use, transfers, gait patterns, and safety to maximize ADLs.
• Documents evaluations, plans, and treatments in medical records; tracks progress and updates plans accordingly.
• Participates in IDT meetings; collaborates with team; supports 24-hour care delivery decisions.
• Maintains professional affiliations, required certifications, and continuing education.
