Job summary

Blue Ridge Care is seeking a Physical Therapist to join its PACE program, providing evaluation, treatment, and ongoing therapy across the PACE Center, participants' homes, and other care settings. You will develop individualized plans and work with an interdisciplinary team to support independence and safety for older adults.

Responsibilities include comprehensive assessments, goal setting, education for patients and caregivers, and ensuring compliance with regulations.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist license.
  • Current CPR certification.
  • Minimum of one year of experience with frail, elderly, or medically complex populations preferred.
  • Experience in interdisciplinary healthcare settings preferred.
  • Strong assessment, clinical reasoning, and care planning skills.
  • Ability to communicate with participants, families, providers, and team members.
  • Knowledge of adaptive equipment, mobility training, rehabilitation, and fall prevention.
  • Valid driver's license and reliable transportation.
  • Background screening and health clearance required.

Responsibilities

  • Conduct comprehensive physical therapy assessments for new participants at enrollment.
  • Perform reassessments at least every six months or as clinically indicated.
  • Collaborate with the Interdisciplinary Team to develop and implement care plans.
  • Provide PT evaluations, treatments, and interventions in the PACE Center, participant homes, and care settings.
  • Establish individualized treatment goals and modify plans based on progress.
  • Educate participants and caregivers on exercises, transfers, gait training, and safety.
  • Assist with DME, orthotics, prosthetics, and footwear selection.
  • Evaluate equipment for proper fit and function.
  • Supervise and mentor PT Assistants and staff.
  • Maintain documentation and required reports.
  • Support quality improvement and regulatory compliance.
  • Maintain equipment and therapy areas in a safe, clean condition.
