Job summary

BoldAge PACE is seeking a Physical Therapist to plan, coordinate, and deliver therapy services for program participants in various settings, including center, clinic, or home. The role integrates with an interdisciplinary team to promote independence and quality of life for older adults.

The candidate will assess equipment needs, develop treatment plans, document progress, and educate families while maintaining high standards of safety and compliance with policies.

Qualifications

• Advanced degree in Physical Therapy from an accredited school.
• Current license to practice Physical Therapy in the state.
• Current driver’s license and proof of auto insurance.
• Two years of experience working on an interdisciplinary team is preferable.

Responsibilities

• Plan, organize and implement Physical Therapy services for PACE participants.
• Assess need for adaptive equipment, specialized shoes and prosthesis; recommend home modifications and equipment.
• Administer therapy plans and document evaluation, assessment and treatment per policy.
• Educate participants and families in procedures, home programs and safety.
• Train PACE staff in body mechanics, transfers, and activities of daily living.
• Maintain documentation in participants’ medical records and participate in IDT meetings.
