Job summary

myPlace Health is seeking a dedicated Physical Therapist for our PACE program in California. You will be part of an interdisciplinary team, delivering personalized therapy plans, leading exercise programs, and conducting home assessments to support mobility and independence for older adults.

This onsite role requires CA PT licensure, CPR/First Aid within 60 days, valid driver’s license, and a Monday–Friday schedule.

Qualifications

• Graduation from an APTA-accredited Physical Therapy program.
• Active Physical Therapy license in California.
• CPR and First Aid certification within 60 days of employment.
• Valid California driver’s license and reliable transportation.
• Ability to lift up to 50 pounds.

Responsibilities

• Collaborate with the IDT to complete assessments and develop care plans.
• Assess mobility, physical condition, and functional abilities to tailor therapy.
• Deliver therapeutic exercises, gait training, and ambulation support.
• Design engaging therapy programs focused on maintenance and rehab.
• Document interventions accurately and follow physician orders.
• Conduct home assessments and deliver in-home treatments.
