Job summary

BoldAge PACE seeks a dedicated Physical Therapist to support participants in maintaining independence at home and within care settings. You will assess needs, develop treatment plans, and coordinate with the PACE interdisciplinary team to deliver high-quality services.

Responsibilities include in-person assessments, home safety evaluations, equipment recommendations, and family education. You will train staff on safe handling and contribute to ongoing quality improvement while upholding OSHA

Qualifications

• Advanced degree in Physical Therapy from an accredited school.
• Current license to practice Physical Therapy in the state.
• Two years of experience working on an interdisciplinary team preferred.
• Bilingual in Hmong preferred.

Responsibilities

• Plans, organizes, and implements Physical Therapy services for PACE participants.
• Performs in-person assessments at initial enrollment and ongoing as determined by the IDT.
• Assesses need for adaptive equipment, home safety, and equipment recommendations.
• Educates participants and families in procedures and home programs.
• Trains PACE staff in body mechanics, transfers, and safety.
• Collaborates with the interdisciplinary team to develop a comprehensive care plan.
• Maintains documentation and participates in quality improvement.
