Job summary

BoldAge PACE is seeking a Physical Therapist to plan, organize, and implement physical therapy services for participants enrolled in the PACE program. You will assess, plan, and provide treatment while teaching participants and families to sustain community living and independence.

Collaborating with the interdisciplinary team, you will ensure quality care, complete documentation, and participate in ongoing education and safety training.

Qualifications

• Advanced degree in Physical Therapy from an accredited school.
• Current license to practice Physical Therapy in the state.
• Two (2) years of experience on an interdisciplinary team in hospital, nursing home or community-based setting preferred.
• One year of experience with frail or elderly population preferred; training available if not present.

Responsibilities

• Plans, organizes, and implements Physical Therapy services for PACE participants.
• Conducts assessments, plans treatment, and teaches participants/family to maintain support in the community.
• Collaborates with the IDT to develop and adjust care plans and documents outcomes.
• Trains staff in safety, transfers, gait, and ADL techniques to maximize independence.
