Job summary

PACS Group is seeking a Staff Physical Therapist to evaluate residents, design treatment plans, and improve mobility and independence in accordance with physician orders and facility policy. You will communicate with families, physicians, and the health team and supervise PT assistants.

You will lead in-services, maintain timely documentation, ensure infection control and safety, educate residents and caregivers on exercises and fall prevention, and support discharge planning.

Qualifications

• Bachelor's degree in Physical Therapy.
• Licensed as a Physical Therapist in the state.
• Proficient with Net Health and Point Click Care.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans.
• Supervise PT assistants in resident care.
• Communicate with physicians, nursing staff, residents, and families.
• Maintain timely, accurate, and compliant documentation.
• Provide in-services on training programs for staff.
• Maintain a safe treatment environment with infection control.
• Educate residents and caregivers on home programs and fall prevention.
• Recommend equipment and assist with discharge planning.
• Maintain confidentiality of resident and company information.

Education
