Qualifications

• Minimum of two years of experience in a hospital, nursing home, or community-based setting.
• One year of experience working with frail or elderly population preferred.

Responsibilities

• Plans, organizes, and implements Physical Therapy services for PACE participants.
• Documents evaluation, assessment, plan, and treatment rendered accurately.
• Educates and instructs participants and families in home programs.

Skills

Bilingual and fluent in Hmong

Ability to assess home safety and mobility

Experience with elderly population

Education

Advanced degree in Physical Therapy from an accredited school

Current license to practice Physical Therapy in the state
