Job summary

Habitat Health is seeking a Physical Therapist to deliver evaluated and individualized therapy across acute, restorative, and maintenance levels for PACE participants. You will collaborate with an interdisciplinary team, develop treatment plans, and conduct home visits as needed.

Required CA license, 1+ year in geriatric care, CPR/BLS, and strong documentation skills. Competitive base salary with a 5% target bonus and comprehensive benefits.

Qualifications

• Bachelor’s Degree in physical therapy.
• Active Physical Therapist license issued by California (CA).
• Minimum of one year experience with frail or elderly.
• Current CPR/BLS certification.

Responsibilities

• Provides physical therapy evaluation and treatment on an acute, restorative and maintenance level to program participants.
• Participates in Interdisciplinary Team by contributing insights from assessments, care plan recommendations, and care coordination.
• Assesses condition, creates a treatment plan and discusses implementation of therapy with participants and caregivers.
• Makes home visits as needed and documents activities in participant’s chart.
• Educates participants and family members on exercise programs and DME use.
• Documents therapy services and maintains department record-keeping within requirements.
• Oversees DME inventory and participates in program development and quality audits.
