Job summary

Qualifications

• Active Physical Therapist license issued by California.
• Bachelor’s Degree in physical therapy.
• Completion of a physical therapy program accredited by APTA or CMS-approved authority.
• A minimum of one year experience working with the frail or elderly.
• Current CPR/BLS certification.
• Ability to thrive in a fast-paced, evolving environment with adaptability and a continuous improvement mindset.
• Bilingual: Spanish/English strongly preferred; other languages a plus.

Responsibilities

• Provide physical therapy evaluation and treatment across acute, restorative and maintenance levels for PACE participants.
• Participate in Interdisciplinary Team (IDT) by contributing insights from assessments and care coordination.
• Create and discuss treatment plans with participants and caregivers, and implement plans.
• Conduct home visits as needed and document activities in participant charts.
• Educate participants and family members on proper care and use of exercises and DME.
• Maintain department records and ensure compliance with documentation requirements.
